DECLARATION

The employer acknowledges that the information provided herein is correct and accurate by completing and submitting this form to Apex HR. The employer agrees that this information will be used by Apex HR to prepare the employment agreement and that preparation times may be impacted if the submitted form is incomplete. The employer agrees that if it is an Apex HR real estate HR subscriber Apex HR will be paid a service fee of $95 per employment agreement prepared by Apex HR. For employers without a current Apex HR real estate HR subscription, the service fee will be $600 per employment agreement prepared by Apex HR. The service fee will be incurred at the time the Request for Employee Agreement Preparation form has been received by Apex HR by email.
